Where do these percentages come from?

The plan covers 100%, 60% or 40% of eligible fees, depending on adjusted family net income. The income bands are under $70,000; $70,000–$79,999; and $80,000–$89,999 respectively. This calculator does not check eligibility.

Your coverage percentage applies to eligible fees, up to the CDCP established fee. Your dentist may charge a different amount. You may owe a co-payment plus any amount above the CDCP fee.
